The fine print
- The membership is for patients without dental insurance and can't be combined with any insurance plan.
- Valid only at Earlham Family Dentistry.
- Plans run for 12 months and are paid in full when you join. You can renew at the end of your 12 months.
- Memberships are non-refundable and non-transferable, including if you don't use your visits.
- Deep cleanings (scaling and root planing) aren't included, but members get the 15% discount on them.
